Acknowledgments

We would like to express our gratitude to Dr. David DeMaso and Dr. Joseph Gonzalez-Heydrich for allowing us to incorporate these families’ voices into their well established Experience Journal model. In addition, their hard work in identifying funding to allow this project to come to fruition was indispensable.  Both are child and adolescent psychiatrists in the Department of Psychiatry at Children’s Hospital Boston and Harvard Medical School.  Dr. DeMaso, Project Director, and Sneha Rao, Project Coordinator, have worked collaboratively with Laurie Glader MD, Carol Nolan RN, Susan Shanske LICSW, and Susan Riley DPT from the Cerebral Palsy Program at Children's Hospital Boston to create this Experience Journal for families. The Cerebral Palsy Experience Journal was made possible by contributions from the Department of Psychiatry at Children's Hospital Boston.

This project could not have been possible without the additional support of a team of reviewers including Stuart Goldman MD, Rick Bourne JD, Elizabeth Wharf LICSW PhD, and Caitlin Bright. We would also like to thank the students, Elisabeth Modestino, Carlyn Burdick, Melissa Alexander, and Amy Hardcastle for their help transcribing, organizing, and editing the content.

The video clips of the family interview were produced and edited by Dixon Yun. The web site was designed by Meghan Toczko. Content was uploaded to the site and organized by Timothy DeMaso.

Finally, and most importantly, we extend enormous gratitude to all of the families who generously provided their stories for the Cerebral Palsy Experience Journal.
